BSN PROGRAM EYE REFERENCE I. Physical Examination A. GENERAL SURVEY 1. Physical appearance: “appears stated age, no signs of acute distress present” Vs. “appears older/younger than stated age” & “general diaphoresis noted, Patient clutching chest, nasal flaring & expiratory wheezing audible, etc.” (cardiac/respiratory signs); “facial grimace, holding body part (e.g., abdomen)” (pain signs) 2. Gender: “sexual development appears appropriate for gender & age” Vs. “delayed or precocious puberty” 3. Level of consciousness (LOC): “A&Ox3 to person, place, & time/date; converses appropriately” Vs. “Unable to state (name, place, &/or time/date)” & “Patient somnolent but able to open eyes & make eye contact when name called in loud voice.
