PROJECT OVERVIEW
Sasol are world leaders in the production of oil and gas from coal.
Their plants contain hundreds and thousands of kilometers of piping – most of which contain potentially
hazardous gases and fuel in transit.
In the mid 80’s, in Sasol’s worst industrial accident ever, 22 plant workers lost their lives when a contractor
used a blow torch to cut into the wrong pipeline.
This is the backdrop to a detailed, process driven, administration loaded, permit to work system. But is the
system as perfect on the ground as the plant records might suggest on paper?
This program was designed to bring the issues around permit to work to the fore at Sasol and to highlight
the potential Achilles heel of an otherwise near perfect system:
