SAMPLE FOR INVITING RESIDENT OR CLIENT REP
Dear Resident and/or Designated Representative:
In order to maintain the continuity of care while also providing a forum of continued improvement, our company
schedules regular care conferences in our offices (or the residence) with residents of assisted living programs,
persons receiving care in their homes, adult day care program participants and their families. We review all of the
care services in advance and prepare an outline to lead the discussion; however the most significant part is what we
hear from you.
The conference has been scheduled in our offices as follows:
