Determining Whether a Student Qualifies as an Individual with a Disability i. Step 1: Has the Parent/Student provided the following documentation: 1. Documentation from medical provider regarding impairment from the last 3 years; 2. Individualized Education Program (IEP) from the most recent school year; 3. IEP Evaluation from within the last 3 years; or 4. 504 Plan from the most recent school year. If the parent/student has not provided such documentation, then notify the parent that the request for support cannot be approved without documentation review. ii.
